Please note: Some questions may require more than one answer for those with multiple stations.  Kindly provide the information by call letter for those questions.

1. Please list all your Station Call Letters and their FCC ID (FID):
2. What is the after-hours Contact number for these station(s)?
3.  What is the after-hours email address for these stations?
4. Primary city of coverage (not city of license)?
5. Are your station(s) EAS LP-1 or LP-2? (call letters for LP1 or LP2)
6. Your station(s) EAS Region? (i.e. Huntsville, Dothan, Mobile, etc.)
7. Do your station(s) have a disaster recovery/business continuity plan?
8. Daily hours that your station(s) has operations staff on site?
9. Daily hours that your station(s) have news operations staff on site?
10. Which stations have technical support staff on site?
11. Are the station’s studios and transmitters co-located?
12. Do your studios have an emergency power generator? If Yes, how long can you run this site before re-fueling is required?
13.  What are your fuel requirements for this site?
14. Does your main transmitter site have an emergency power generator? If Yes, how long can you run this site before re-fueling is required?

16. Do any of your stations have alternate transmitter sites? If Yes, is the alternate transmitter site full power/full coverage?
17. Do any of your stations have an alternate or emergency studio location? If Yes, does this alternate studio location have an emergency power generator?
18. What are your fuel requirements for this alternate site?
19. For each station, is the tower Self Supporting, Guyed or on a building roof?
20. What is the approximate age of each station broadcast tower?
21. How often are your emergency power generators and alternate transmitter sites tested?
22. What is your stations’ connectivity to the public internet for emergency information gathering and EAS/CAP/IPAWS?
23. Do your stations have an alternate public internet connection? If Yes, is this connection from a wired provider (Cable or Telco) or wireless provider (Cellular or Satellite)?
24. Do your stations have a direct wireless connection (Dedicated 2-way or HF radio) to local emergency management?
25. Have any of your stations partnered with a local Amateur Radio club for backup communications?
